Laser Resurfacing vs. Microneedling: Choosing the Right Skin Renewal Treatment

Dr. Renzo, Clinical Aesthetics and Laser Specialist
October 26, 2026

The choice between laser resurfacing and microneedling depends on what is causing the concern, the exact treatment proposed and the balance of benefit, recovery and risk. “Laser” includes several technologies, so a useful comparison needs more detail than a category name.

Start by naming the concern

Depressed acne scars, fine lines, uneven texture and dark patches do not all need the same treatment. A clinician should assess the diagnosis, scar pattern, skin tone and healing history before recommending a procedure. A new or changing pigmented lesion needs appropriate evaluation rather than cosmetic treatment based on appearance alone.

Understand the three approaches

  • Conventional microneedling creates small punctures to stimulate a repair response.
  • RF microneedling adds radiofrequency energy delivered through needle electrodes.
  • Laser resurfacing uses light energy. Ablative methods remove treated surface tissue; nonablative approaches heat tissue without removing the skin surface. Fractional treatment can be ablative or nonablative.

AAD microneedling guidance and Mayo Clinic laser guidance explain these categories. IPL is a separate light treatment, not a laser.

What does comparative research show?

A 2026 analysis of eight randomized acne-scar trials found greater average scar improvement with fractional CO2 laser than RF microneedling, alongside more pain, post-inflammatory darkening and longer redness. These findings concern the studied acne-scar treatments. They do not establish that every laser is better than conventional needling or that the same tradeoff applies to every rejuvenation goal. Read the comparative review.

Compare risks as carefully as benefits

Laser treatment can cause lasting pigment changes and scarring, among other complications. RF microneedling requires its own discussion because the FDA has reported burns, scarring, fat loss, disfigurement and nerve injury with certain uses. Conventional microneedling also has risks, including infection, pigment changes and scarring; “less aggressive” should not be read as risk-free. FDA RF safety notice.

Ask about the treating clinician's experience with the device and your skin tone. Review alternatives and what happens if the initial response does not justify further treatment.

Make recovery concrete

Ask when you might return to work, wear makeup, exercise and resume active skincare. A treatment with fewer visible effects may still be uncomfortable, and a short appointment does not guarantee a short recovery. Plan around travel and outdoor activities rather than an exact promised finish date.

Bring the eight-question consultation checklist and review the CO2 recovery guide if that modality is proposed.
